Lincolnshire Prairie View School District 103 issued the following announcement on July 25.

Lincolnshire-Prairie View School District 103 staff hopes this email finds you enjoying summer with your family and friends. If you haven't done so already, please turn in your completed medical forms to your child's school. Forms are available on the district website or from your doctor's office.

Required for incoming kindergartners:

• Completed physical exam form with verification of immunizations
• Completed dental exam form
• Completed eye exam form (New students moving into the district from another Illinois district may disregard this form.)

Required for incoming second graders:

• Completed dental exam form

Required for incoming sixth graders:

• Completed dental exam form
• Completed physical exam form with verification of immunizations

Required for students new to D103:

• Completed physical exam form with verification of immunizations
• Completed dental exam form
• Completed eye exam form (New students moving into the district from another Illinois district may disregard this form.)

Proof of state mandated physicals, exams, and immunizations are due to the D103 nurses by Monday, August 6. D103 does not allow children to attend school until medical paperwork is on file with the nurse. Your prompt attention to these requirements will help provide a smooth transition to school for your child.
