SIHF Healthcare is set to provide school physicals, sports physicals, and immunizations for all students of Collinsville Community Unit School District 10. The event will take place at Collinsville High School on Tuesday, July 22, and Wednesday, July 23.
Clinicians will be available from 11:00 am to 5:00 pm on both days. Those interested can call 618-578-5701 for more information or to schedule an appointment. While appointments are recommended, walk-ins are also accepted.
SIHF Healthcare accepts insurance but it is not mandatory for participation in the event. Consent forms can be completed online.
