Granite City Community Unit School District No. 9 is collaborating with the Madison County Health Department to organize Back to School Physical and Immunization Clinics. These clinics are scheduled for Thursday, July 31, and Thursday, August 7, from 1-4 p.m. at the Granite City High School Annex Gym.

The purpose of these events is to ensure students meet school and sports physical requirements. All incoming kindergarteners, sixth graders, and high school freshmen must have a school physical and updated shot records.

The clinics will be held at the Granite City High School Annex Gym located at 3101 Madison Ave. The event is open to preschool through twelfth-grade students. While appointments are preferred, walk-ins are welcome. Patient insurance, including public insurance such as Medicaid, will be billed for services. There will be no out-of-pocket fees for eligible participants.

Participants should bring their child along with a parent or guardian's ID, shot/immunization record, and insurance card. Those without insurance or who have questions about vaccines can call 618-692-8954, Ext. 2.

Appointments can be scheduled online at https://form.jotform.com/251473904886165.
