Our Community Care Connection (CCC) has received great news — an additional $1 million as part of NorthShore University HealthSystem's Community Investment Fund, which brings our partnership with NorthShore to $1.7 million!
We look forward to strengthening this partnership, and are grateful to be teaming up with NorthShore as one of the 2023 Community Investment Fund partners. Together, we will further expand our CCC services and team, as well as contribute to good patient outcomes that enhance healthcare access throughout Lake County’s underserved communities.
