Round Lake schools set October deadline for student health forms

Round Lake Community Unit School District 116 has announced that physical and health forms for students are due by October 15. The requirement applies to all students in PreK, Kindergarten, sixth grade, and freshmen, as well as any student new to Illinois. These students must submit a physical form dated within the past year. Additionally, all students in Pre-K, Kindergarten, sixth grade, and seniors need updated health forms.

The district stated that "Any student who does not have this paperwork turned in by October 15 WILL be excluded from school starting October 16 until the required paperwork is turned in." Forms can be submitted directly or emailed to the child’s building nurse. They may also be faxed from a doctor’s office to (847) 512-8473. Appointments for physicals are available at Round Lake High School; families can call (847) 377-8800 to schedule. For questions, parents are advised to contact their student's building nurse directly.

The announcement is also available in Spanish: "¡La fecha límite para entregar las formas estatales del es el 15 de octubre!... Cualquier alumno que no haya entregado estos documentos antes del 15 de octubre NO PODRÁ entrar a clases desde el 16 de octubre en adelante hasta que haya entregado los documentos requeridos."

Round Lake Community Unit School District 116 includes several schools across Lake County such as Indian Hill Elementary School, Magee Middle School, Pleviak Elementary School, Raymond Ellis Elementary School, Round Lake Beach Elementary School, Round Lake Middle School, Round Lake Senior High School, Village Elementary School, and W.J. According to data from the Illinois State Board of Education, the district employs 512 teachers with an average salary of $69,620 before pension contributions; about 72 percent are women and nearly 28 percent are men. No teachers reported more than ten absences during a school year (https://www.illinoisreportcard.com/).

Demographically, the district serves a diverse student body: approximately 77.7 percent Hispanic, 11.9 percent White, 6.7 percent Black, and one percent Asian (https://www.illinoisreportcard.com/). In fiscal year 2020, spending per student was $24,469 with total expenditures reaching $162 million (https://www.illinoisreportcard.com/).
